· Patients who require travel health advice will be asked to complete the “Travel Risk Assessment Form” when calling to book appointment. This form needs to be handed into the surgery AT LEAST 10 weeks before their travel.
Our nurses will need to review your medical record and then they can provide travel health advice and arrange for appropriate immunisations. Some vaccines need time to take effect and some may require a course over several weeks.
